We are a trusted place for free, unbiased information about long-term services and support in New York State for people of all ages or with any type of disability.
Home Health Aides (CDPAP)
This is a New York State Medicaid program that allows home care consumers to recruit, hire, and direct their own caregivers – including most family members. Through CDPAP, loved ones can get paid as a caregiver for their family and friends.
Long Term Care Ombudsman
The Ombudsman Program is an advocate and resource for older adults and people with disabilities who live in nursing homes, assisted living, and other licensed adult care home
Benefits Advisement
We assist people with any questions you may have about accessing and using Social Services benefits, including Medicare, Medicaid, and Social Security (SSI and SSDI).
Vision Services
We assist people who are blind, legally blind, or visually impaired with attaining or maintaining personal independence with techniques available at your home.
Housing Transitions
We assist people in nursing homes access the services they need to return to and live in the community.
Advocacy/ Education
We support the Independent Living movement through local partnerships and coalitions and education about issues impacting people with disabilities.
Public Accessibility
We are a contact for ensuring compliance with public accessibility requirements.
Peer Counseling/ Other Services
We provide support and resources to address the life challenges that many people face, including job placement, medical transportation, housing issues and peer support.
